 Completed JRDA Membership Application
 Application Fee of $150
 Completed JRDA Confidentiality Agreement by Officer
 Roster of at least 10 skaters meeting regularly -Information to include: Legal Name, DOB, Gender
Forms Mailed to: JRDA, PO Box 170427, Austin, TX 78717
Membership Length is One Calendar Year, except year 1 of JRDA Membership
 *Inaugural Year* (June 1, 2010 - December 31, 2011)
