Players participating in level 3 / full contact practice or play, that are under the age of 14 as of September 1 of that season will need to fill out the age waiver with their guardian and coach.

The JRDA membership season is from September 1 - August 31

Visiting participants (skaters, coaches, and officials) from another league will need to fill out the liability waiver for games and mixers played at your league's "home" facility. You may want to download a copy of this form as they will need to be filled out for each event during the season.
